Primary HealthCare offers nutrition services to promote healthy lifestyle across the life-course through better nutrition.
Services include:
- Weight Management Programmes in various Health Centres and community clinics;
- Health talks in the community, including schools, day care centres, workplace settings, organizations and local councils;
- Health talks on the media;
- Information sessions on healthy eating at public events;
- Providing nutrition support to individuals on general healthy eating over the phone.
Referral Guideline
A patient should be referred to a nutritionist when it would be beneficial for the patient to be in a group of people, to learn more about healthy eating and different recipes, but the patient will not have a one to one consultation and meal plan.
A patient should be referred to a dietitian when the patient needs a one to one consultation during which the dietitian will review and consult latest blood results from the health centre file, upon which a meal plan is drawn up centred around the patient's dietary needs, health conditions and cooking skills.
