Application form

Section A : PERSONAL INFORMATON 
All the information is to be filled in by the Employer or Employee and signed by BOTH. Please make sure to attach a copy of the relevant course as approved by MQRIC.

Section: B & C: HEALTH SCREENING & INFORMATION FOR MEDICAL DOCTORS 
All the information is to be filled in by a Local Private Medical Doctor.

Section D: APPLICANT’S DECLARATION 
Signed by the applicant and dated.

t is important that all fields of the application form are duly and legibly filled, signed, dated, and submitted by the employer to workpermit.idcu@gov.mt for processing.

Health Screenings requirements.

a)  
Chest X Ray
 •  Chest X Ray is required for applicants who were born or have lived for 6 months or more in a country reported as high-risk for tuberculosis - (list attached​)
 •  Chest X-Rays need to be taken within the last 6 weeks from the date of this application.

Applicants who are changing jobs do not need to repeat the Chest X Ray if this was taken locally less than a year ago. If Chest X Ray was taken more than 1 year ago, a repeat of Chest X Ray is required. 

If results show any abnormalities, a copy of the Chest X Ray report needs to be attached with the application form. 

b)  
Blood Tests – (to be taken before the administration of the vaccines)
 •  Tuberculosis (Interferon-Gamma TB test)
 
 •  Hepatitis B (Antigen (HBsAg) - 
needs to be taken immediately prior to initiating Hepatitis B vaccination schedule.

 •  
Hepatitis B antibody (anti-HBs) - to be taken only if Hepatitis B vaccines were not completed in Malta or if Hepatitis B vaccines were taken more than 10 years from the date of application.


 •  Hepatitis C antibody (HCV)

 •  Measles Antibody titre result (IgG measles) - All employees must present blood test for IgG Measles. This can be omitted if employee received Measles vaccines in Malta. Should the IgG Measles show low immunity, it is strongly recommended for the applicant to receive a booster of Measles vaccination. Vaccinations taken from any other countries are no longer accepted for processing.

c)  Vaccines

All vaccines to be administered in Malta

Hepatitis B:
 •  Applicants need to follow the Hepatitis B vaccination schedule as highlighted on the application form.
 
 •  
Applicants need to have taken at least the 1st dose of the Hepatitis B vaccine for the application to be processed. The full course needs to be taken before renewal is approved. Without it, renewal of work permit will not be issued.

Measles and Polio:
All employees must present blood test for IgG Measles. This can be omitted if employee received Measles vaccines in Malta. 
Should the IgG Measles show low immunity, it is strongly recommended for the applicant to receive a booster of Measles vaccination. 

All employees (irrespective of the job applying for) are required to take 1 dose of Polio vaccination in Malta.


Covid-19:
Vaccination certificate to be attached
2 doses are strongly recommended.
